What do I want the clinician to understand?

Describe your health concern using the words you would say aloud. Add what you hope would change and what feels uncertain.

List medicines and supplements I currently use.
Note allergies, past reactions, and any pregnancy or breastfeeding questions.

What do I want to know about the medicine?

Keep the name from the advert or offer beside the name the clinician discusses. Ask about any difference.

Ask for the ingredient name and form.
Ask what condition the product is intended to treat.
Ask why this product fits my situation.
Ask what approved alternatives the clinician considered.
Ask what is known about benefits, harms, and gaps in the evidence.

What do I want clear before paying?

Write the replies beside each question. If a figure is a starting offer, ask what follows it.

Get the clinician’s full name and role.
Get the pharmacy’s exact name.
Ask for the full cost of the first medicine supply and each later supply.
Ask about insurance paperwork and any recurring fee.
Read the cancellation and refund terms.

What needs a plan after the visit?

Ask for the follow-up plan in plain words. Leave room to write who answers questions after the medicine arrives.

Ask who explains the medicine label and storage.
Ask who to contact about symptoms or a confusing label.
Ask how tests or an in-person examination would be arranged.
Ask how my regular doctor can receive the visit report.
